It feels like a beauty hack, and honestly, it is! But here’s the reality check: Lip blush is a medical-grade cosmetic tattoo. Because we are using needles to deposit pigment into your skin, your internal health is just as important as the external color we choose. Sometimes, I have to be the “beauty buzzkill” and ask for a doctor’s note before we start.
If I ask you for medical clearance, please know it’s not because I’m trying to be difficult. It’s because I want you to heal beautifully and, more importantly, stay safe. Let’s dive into why your health history matters and which conditions require a “thumbs up” from your physician.
Why Does My Health Affect a Lip Tattoo?
You might think, “Ambar, it’s just my lips! Why does my heart medication or my thyroid matter?” Your skin is your body’s largest organ, and it’s the primary barrier of your immune system. When we perform lip blush, we are creating “micro-trauma.” To heal that trauma and “lock in” the pigment, your body needs to be in tip-top shape. If your system is busy fighting an autoimmune flare-up or dealing with thinned blood, the results can be patchy, blurred, or even dangerous.
Conditions That Require Medical Clearance
If you have any of the following, I’ll need a signed note from your doctor stating that you are cleared for a permanent makeup procedure.

| Recent Surgeries | If you’ve had surgery (including joint replacements) in the last 6 months, your body is in recovery mode. Some doctors also require “pre-medication” antibiotics for dental or tattoo work after certain surgeries. |
| Chemotherapy/Radiation | We generally need to wait at least 6-12 months after your final treatment. Your immune system needs time to bounce back so you can heal safely. |
| Heart Conditions | Specifically anything requiring a pacemaker or history of endocarditis. We need to ensure the procedure won’t interfere with your heart health or medications. |

The “Blood Thinner” Dilemma
This is the one I encounter most often. Many people take a daily baby aspirin or supplements like Fish Oil and Vitamin E. While these are great for your health, they make your blood “slippery.”
If you are on prescription blood thinners, do not stop taking them just for a beauty appointment! That is incredibly dangerous. Instead, talk to your doctor. They may give you a window where it’s safe to skip a dose, or they may tell you that permanent makeup just isn’t a good fit for you right now. I will always defer to their expertise.
Cold Sores & Lip Blush
While not a “doctor’s note” situation in the traditional sense, if you have ever had a cold sore (even just once ten years ago!), you must be on an antiviral medication (like Valtrex) before your appointment. The friction of the needle can trigger a massive outbreak, which will ruin your pigment and cause a lot of discomfort.

How to Get Your Clearance
It doesn’t have to be a complicated process! Most doctors are happy to provide a simple note via their patient portal. You can simply ask:
“I am planning to get a lip blush (cosmetic tattoo) procedure. Because I am managing [Condition/Medication], my artist requires medical clearance to ensure I am safe for the procedure and will heal properly. Do you have any concerns or specific instructions for me?”
